C. Laboratory Authorization

1. Initial Authorization of Radionuclide Laboratories

A completed A-4, Application for Radionuclide Laboratory Approval, form must be submitted to the RSO for each laboratory in which a PH will be using or storing unsealed forms of radioactive material. (This includes those labs which will be shared with another PH and counting rooms). Upon receipt of the completed A-4 form, a member of the RSO may perform a site visit of the laboratory to verify that the information provided is accurate and to assure that the laboratory is properly equipped for the intended radionuclide use. Posting and labeling requirements may also be evaluated during the site visit.

When the previously mentioned items have been addressed, the PH will receive a memo indicating preliminary approval for that specific laboratory. Formal approval and authorization of radionuclide laboratories will take place at the next quarterly meeting of the RRSC. (The exception to the RRSC review may be the addition of "specific-use areas" in labs that have been previously approved.) The PH will receive written notification that the RRSC approval has been granted through the issuance of a revised A-8, Radionuclide Use Permit, form. The PH should review the permit to verify its accuracy and assure that all "Conditions of Authorization" are followed.

a. Shared Laboratories

It is acceptable for an authorized radionuclide laboratory to appear on more than one permit (i.e., the lab may be utilized by more than one PH); however, each PH is responsible for surveying the lab. If desired, one PH may be designated to survey the lab, but each PH must retain a copy of the results in his/her Radionuclide Inventory and Survey Book. In addition, the survey must include all areas where radioactive material is used or stored.

b. Specific-Use Area(s)

A PH may designate a lab as a "specific-use area" on his/her permit. A specific-use area is a lab in which the PH will be using one piece of equipment (e.g., centrifuge, refrigerator, etc.) for radioactive material use or storage. The only exception to allow the use of more than one piece of equipment in a specific-use area is for counting equipment. In other words, a PH may define a lab as a specific-use area and use multiple pieces of counting equipment (e.g., LSC and gamma counter). With respect to specific-use areas, the PH is still required to survey the area in accordance with his/her permit; however, he/she is not responsible for other areas in the lab. If a PH wishes to use other areas in a specific-use area lab for radioactive material work or storage, he/she must submit a revised A-4 form prior to use.
