C. Laboratory Authorization
2. Modification of Authorized Radionuclide Laboratories
a. Major Modifications
The RSO must be notified before any major modifications (e.g., remodeling or renovation) are made to authorized radionuclide laboratories. Appropriate surveys must be performed by the RSO to assure that no significant contamination or radiation levels exist which might result in exposure to individuals performing the laboratory modification (e.g., Campus Facility Services personnel or outside contractors). Before radioactive material use is resumed in a modified radionuclide laboratory, an updated A-4, Application for Radionuclide Laboratory Approval, form must be completed and submitted to the RSO.
b. Minor Modifications
Minor modifications (e.g., relocation of a lab bench) or addition of new equipment (e.g., refrigerator) do not require the submission of an updated A-4 form; however, a new floor plan should be forwarded to the RSO.
