N. Radionuclide Inventory and Survey Book

A Radionuclide Inventory And Survey Book is provided to each PH to file pertinent radiation safety records. These records are reviewed by the RSO during laboratory audits. Lab personnel should be familiar with the book and its location. It should also be readily available to all lab personnel and the RSO staff. Radiation safety records should be kept for a period of three years. The book is divided into the following sections:

  1. SURVEY INSTRUMENTATION - Survey meter and liquid scintillation/gamma counter information, such as instrument efficiency, may be filed in this section.
  2. INVENTORY - Inventories must be kept for each approved radionuclide. It is not necessary to keep a separate inventory for each chemical form. These records should be kept to the nearest microcurie (mCi) and updated after each transaction (e.g., receipts, waste disposals, transfers). Inventory records may be maintained on a computer provided a hard copy is placed in the survey book. Records may also be posted on refrigerators, but should be indicated as such in the Radionuclide Inventory and Survey Book.
  3. SURVEYS - All required direct and/or contamination (wipe) surveys should be recorded on a Radiation Survey Data Sheet and filed. Surveys should be performed at the frequency required by the PH=s permit.
  4. RSO INFORMATION - Radiation Safety Newsletters and other RSO information (e.g., Radionuclide Use Permit) should be kept in this section. Everyone must read the distributed information before it is filed.
  5. MISCELLANEOUS - Memos from the RSO, blank forms (e.g., A-13 forms, A-5 forms), etc. may be kept in this section.
