Waste Pick-Up Request

To schedule a waste pick-up, please complete the following form in its entirety.  The Waste Schedule July-December 2009 should be used to determine when we will picking up waste in your building.

Is your waste ready for pickup, including waste tags on ALL containers, wipe survey results acceptable, and a shipping form completed?

              

* If "No," STOP HERE!  Do not proceed with scheduling your pickup until all required items are completed.  Any pick up request received with "No" checked will be rejected.
